The following AUTHORIZATION FOR FEMALE RECONSTRUCTIVE SURGERY document was donated to me by a former patient of Ms.Vicki Hufnagel. The patient who signed this form travelled to Mexico for treatment because of information obtained online from Ms. Hufnagel's website as well as from FRS "Advocates." The patient's name has been obscured to protect her identity and to protect her from any unwarranted public attacks online.

This is the "informed consent" document provided to patients when they check into Villa Floresta Hospital Medical Center in Mexico. Not exactly what I would have expected given Ms. Hufnagel's extremely vocal stand on the issue of informed consent.
